The Platinum Choice credit card offers competitive rates & terms, no loan documents, no income verification, no employment verification and low credit card rates. The Premiere Program is fast, simple & easy with quick decisions in establishing high credit lines. The program also offers very low, flexible minimum credit card monthly payments that decrease with each monthly payment. Listing the digits on your credit card on this application will take the place of running a credit check on this application. Should you not qualify for the Premiere Program, fill out the remainder of the application without entering information in the credit card reference spaces. A customary credit check will then be performed. The ‘ChargeCard’ may be used for multiple procedures. Applicant does not have to be the patient.

To Qualify: For the Premier Program you must have a major credit card such as a Visa, MasterCard, Discover or American Express. Department store, Gasoline and Retail Merchant credit cards also qualify. You are not required to have a credit card to apply for a standard loan.

Restrictions: Debit cards or check cards or PO Box addresses do not qualify.

NOTE: On this application, there is no space listed for the expiration date. Neither expiration date nor the key codes on the back of the credit card should be listed on this application. Your credit card will not be charged, nor will the credit card issuer be notified. A new Platinum Choices credit card will be issued upon approval and your acceptance
